GLP-1 Medications & Lifestyle: Why Nutrition Still Matters

The conversation around GLP-1 medications has exploded over the past year. These therapies are changing the way we think about obesity and weight management, offering new hope to millions of people who have struggled with traditional methods of weight loss. But as employers, providers, and payers weigh the questions of coverage, step programs, and mandatory lifestyle support, one truth remains: there is no replacement for a healthy lifestyle.

The Complex Reality of Weight Loss

Weight loss has never been simple. It involves a complex interplay of genetics, biology, environment, behavior, and access to resources. GLP-1s are powerful tools, but they are not magic. Without attention to nutrition, movement, sleep, stress, and social connection, the long-term impact of these medications can fall short.

New Guidance from Leading Health Organizations

This was reinforced by a recent joint advisory released In May of 2025 by four leading organizations: the American College of Lifestyle Medicine (ACLM), the American Society for Nutrition (ASN), the Obesity Medicine Association (OMA), and The Obesity Society (TOS). Their guidance outlined eight key priorities for supporting patients on GLP-1 therapy:

  1. Individualized initiation of therapy – ensuring patient goals and readiness guide the approach.
  2. Baseline nutrition assessment – understanding diet quality and habits before starting.
  3. Managing gastrointestinal side effects – addressing nausea, vomiting, or constipation with dietary strategies.
  4. Promoting nutrient-dense, minimally processed foods – moving beyond calorie restriction.
  5. Preventing micronutrient deficiencies – especially in vitamins, minerals, and fiber.
  6. Adequate protein intake + strength training – to preserve muscle and bone health.
  7. Maximizing outcomes through healthy eating – leveraging nutrition to enhance medication effectiveness.
  8. Whole-person lifestyle changes – including sleep, stress, substance use, and social well-being.

Beyond the Calorie Deficit

Weight loss conversations often stop at calories in vs. calories out. But the body needs much more than just an energy balance. Specific nutrients—protein, calcium, vitamin D, magnesium, fiber, and more—support every system in the body, from immunity to mental health to bone strength. Without adequate nutrition, weight loss can come at the expense of long-term health.

Why Lifestyle Integration is Non-Negotiable

For organizations deciding on GLP-1 coverage, the key takeaway is this: medication alone is not enough. Pairing pharmacotherapy with structured lifestyle support ensures better outcomes, reduces side effects, and promotes sustainable health. This is not just about losing weight; it’s about building the foundation for long-term wellbeing.

The Path Forward

GLP-1 medications represent an exciting advance in obesity care. But the best results come when they are paired with nutrition and lifestyle interventions that address the whole person. Employers, providers, and health plans have the opportunity and a responsibility to reshape obesity care; not as a choice between medication or lifestyle, but as an integration of both.
